En este artículo se revisan las novedades aparecidas en la última actualización de la estrategia global para el diagnóstico, tratamiento y prevención de la EPOC (GOLD). En ella se enfatiza la importancia de individualizar las decisiones terapéuticas. Incluye dos cambios clave. En primer lugar el tratamiento inicial se separa de la terapia de seguimiento, estableciéndose estrategias de escalada y reducción progresiva del tratamiento. Por otra parte, se introduce el recuento de eosinófilos en sangre como un biomarcador para estimar la respuesta a los CI en la prevención de las exacerbaciones. Es posible que en las futuras actualizaciones se disponga de nuevas evidencias y de opciones terapéuticas más novedosas que conduzcan a mejoras adicionales en el abordaje de los pacientes con EPOC
In this article we review the novelties that appeared in the last update of the Global Strategy for the Diagnosis, Management, and Prevention of Chronic Obstructive Pulmonary Disease (GOLD). It emphasizes the importance of individualizing therapeutic decisions. Includes two key changes. First, the initial treatment is separated from the follow-up therapy, establishing strategies of escalation and progressive reduction of the treatment. On the other hand, the eosinophil count in blood is introduced as a biomarker to estimate the response to inhaled corticosteroids in the prevention of exacerbations. It is possible that in the future updates new evidences and new therapeutic options will be available that lead to further improvements in the approach of patients with COPD
